PLEASE FIX THIS PAPER I HAVE % NON PERFORMANCE ON THIS> Americans with chronic diseases and without health insurance significantly add to the overuse and abuse of hospital emergency rooms (Parker 2002). 45% of uninsured Americans with chronic diseases have income less than 200% of the poverty level (Cover the Uninsured 2009). There has been little change in these numbers over the last decade, and the populations in inner city and rural areas continue to increase the number of people under the poverty level. The financial responsibility of these populations fall on society, and on the institutions where they go for their healthcare. An unbelievable amount of families routinely use the Emergency Room at local hospitals for all care, leaving enormous unpaid balances, which contribute to the rise of healthcare premiums for those who work and carry insurance for themselves and their families.
Having no health insurance also often means that people will not get necessary care and that means chronic illnesses will be prevalent in the uninsured population. Immunizations for children and adults would not be given if not for the free clinics or health departments. The elderly are most likely to be affected, due to the fact they cannot afford the prescription medications monthly, and cannot afford to go to the doctor to get free samples, or ask for help. There is a 49 billion dollar price tag on healthcare costs for the uninsured, and finding a way to offset this enormous drag on the healthcare services industry will always be a problem. There is no way to make sure all citizens are insured and getting the healthcare they need. Even with the passing of the new healthcare laws, there will always be a great number who will not be insured.
The ethical question is, “Do the uninsured deserve the same care as people who pay for their own healthcare”? I see many households who have multiple families living under one roof, who do not work, have no income except for welfare, own no vehicle, and have many uninsured children who need care. How do we try to find a way that helps these families? I think that having mandatory classes for expectant mothers, as well as other members of the family, especially if they are in the welfare system, what kinds of resources are available for healthcare services, discounts, programs, and physicians who would be associated with these programs, and also training available on understanding these programs. It is imperative that we take a look at the education and minority status of the uninsured, and offer a different kind of life strategy, such as skills training, running a household, cooking, understanding a budget, how to shop for groceries, the cost of monthly household expenses and where to find help when they need it. There are so many people who could be doing better, if they fully understand how their household works, and how to obtain insurance for even coverage of yearly exams, and walk them through the process. The advantages to having populations of uninsured citizens fully engaged in their own healthcare, finances, and future are invaluable. Expecting this to happen overnight, or all by itself is ludacris and we must strive to be a community who take care of those who need help in every way, but at what cost?
Delaying care for fear of medical bills is a downward spiral that leads to ultimately higher health care costs for all of us. More than one third of uninsured adults have problems paying their bills, which explains why many of the uninsured don’t seek out the care they need until the last minute. Productive citizens could start a new revolution of health conscious, paying customers for healthcare providers, as well as decrease the number of chronic illnesses and cancers that go undiagnosed. It is sad to think, that because of a prejudice within the healthcare industry against uninsured people, they have chronic conditions that could have been prevented, if only we would take the people more seriously, and not the profit in the industry. There has got to be a place to get care when uninsured, and be treated with respect. How can we create a system that rewards people for plugging in to their own care? Mandatory classes usually do not work in the way they were originally expected to, in helping educate individuals on healthcare, chronic illness, caring for children or themselves. We are in an age where families are living under the same roof with extended family, and with seniors who need caregivers. This is another issue regarding uninsured people, caring for senior populations on a budget who have nothing but Medicare and no additional income to pay for the rising cost of prescriptions and testing. The outcry for affordable or free services to care for chronic conditions such as dementia, Alzheimer, cancer, and diabetes has never been higher. With the senior population at an all- time high, the services offered to the poor and uninsured have not changed, leaving many seniors with the burden of paying more and more healthcare costs. There are less and less clinics, hospitals, and physicians who will take on new Medicare, Medicaid, or uninsured patients. Where can these people go for care is limited, and results in the Emergency Rooms being overfull, and understaffed with exorbitant fees going unpaid. There is an unrealistic expectation of services to uninsured which causes the rural hospitals to get taken over by big corporations, just to stay open and serving their community.
Wherever our healthcare system is headed in the future, we should ask ourselves some important questions along the way. We are voters and we have a role in deciding what our healthcare system will look like in a year or ten years, but we also have a responsibility to figure out what we’re willing to do to get there. Will our society leave behind the indigent, elderly, and poor populations because we cannot afford them and are unwilling to carry the weight of their baggage on healthcare costs? What sort of problems will this cause? This behavior by society will ensure the death of many people who deserve a chance at a better life and future. Big business will always be just that, and we as individuals must decide how we want to treat our brethren, and who we want running out communities, states, and country. There are no easy solutions, but there are ways we can come together and brainstorm a new system, before it is too late. What these solutions are, at this time unknown, and scary but we must do our best to improve our system. Forcing the working class to pay for the poor is unreasonable, and the rich find loopholes that keep them above worry of this problem. Bureaucrats are generally lazy, and will not take the time or resources to try and help this national travesty. We are running out of time, and it seems the coalition between county, state, and federal governments is not going to happen, so it is on the shoulders of those who care. Seeing just how devastating this problem is affecting all people, is a huge step forward to finding the best solution for the rising cost of the uninsured, and changing how society thinks of minority populations is key to this change. I cannot explain to you the way it makes me feel, to see a family get care for their elderly parent, or chronically ill child, when they cannot afford it. My own growing interest in helping people, the homeless, poor, mentally ill, chronically ill, ignorant and uninsured has led me to realize that our systems do not create an environment that is inviting to those who need help. This must change.
Why not provide healthcare insurance for those in this country who cannot afford their own? What we need to think about, is how we would fund this program, who would be eligible, and how would be start this program. A major question in my mind is, “Who decides how many and who would be able to gain entry into such a program”? This type of program may take years to get up and running, and would need state government, local government, federal government, politicians, doctors, hospitals, and insurance companies to cooperate. What doctors do you know that want to work for free? What hospitals can keep running with deficits so high? There are many difficult questions to be answered, but we need to start this change. Can we come together and agree on a starting point? Was the Affordable Healthcare Act the answer? I do not think the laws enacted are the solution. It has created new problems, that may be easier to rethink and handle, but it is not the Hail Mary for this countries uninsured problems. Fixing the problem is not necessarily the whole issue, changing how we think is a key aspect of solving this issue. There have been some new ideas for coverage of the basic healthcare needs for the young and old which might work for each state with this sort of program, such as Michigan. My interpretation of this program is that it helps get these people in to be checked, and then what happens to them is up to them. If they cannot afford further care, these people will not continue treatment with a specialist, or surgeon and leave the condition unresolved. Another problem is how do you qualify for these programs, and who decides how many will be on the states programs? Financial stability play a role in these programs, and generally the poor who are already in the welfare system take front row seats to these programs, leaving those who do work, but do not make enough for healthcare still uninsured, with no coverage.
We face an enormous group of welfare populations, who have proven that they do not have the resources and know how, to keep up on paperwork and monetary funds needed to be in a program. The sad truth is that, there would always be excuses, which would lead the services to again fall on the system, and the people who work and pay taxes. Keeping a program of this sort running would take so many administrative hours, and who would pay for this? Would it be cheaper to pay for all the overhead to a program of this sort, or just keep paying into the system and writing off balances as we are already doing? These questions remain the constant in a problem there is no solution for.
Independent caregivers cannot keep giving care without payment. It is unreasonable to think that any group would be willing to carry balances for uninsured when there are programs they could get into. When patients lack health insurance, they become willing to use emergent services for their benefit, and this drains funds. With a growing medically uninsured population, clinicians are billing differently and lowering standards, turning patients away when they cannot pay for the costs of care. Dilemmas that clinicians face when patients cannot pay for needed medical services besides in billing are where to send them. Clinicians’ responses to these dilemmas impact the uninsured in the current health care communities, and how people are referred to specialty offices for further care. To effect change, however, physicians and their staff must change how they think about these populations outside of the office, and want to help this change.
There have been some states that have tried to enact their own programs for relief of this burden from uninsured populations, but so far none have been able to completely eliminate the problem. Even with the emerging new healthcare laws enacted by congress in the last few years, the issue will not be resolved anytime soon. There is too much paperwork that must be learned in order for uneducated, disabled, or other challenged people to participate in such a new program. Not only, do we need to educate the people, we need to be able to understand where we are going with this program, the shortfalls, the advantages, and the monetary benefits of giving so much up front for this program to work. So far, I believe it is up to the healthcare professionals to come up with a support system that works for these populations, and management of the education included.
One thought might be to have group trainings on the advantages of healthcare, and how the impact could touch your family. But how do we reach the populations that need this? Do we have the classes at DHS, jails, clinics, schools? The decision to take the initiative and run with it, is an enormous one, and very important. Well thought out structure, was not given with the new healthcare system, and it has not completely eliminated uninsured patients, not do I see it becoming a success for this dilemma. Should we just keep letting facilities, and physicians foot the cost of non-payment? Does this mean that I will get mediocre care at a clinic I choose because of the complacency of staff due to the overwhelming numbers of uninsured people using the clinic for their primary healthcare needs? There is evidence of such care in the elderly, minority populations, who have the greatest numbers of uninsured. The sad truth is, there is no way to get a system where everyone is insured, and everyone gets the care they need, without becoming a socialist society where healthcare is an unalienable right. This of course, has its own downfalls. The free medical cards for Medicaid have been abused for years with devastating affects to the number of places excepting Medicaid. Slow payment, or no payment, is what I keep hearing from office managers from Medicaid. This program should be used for those who absolutely cannot work and afford medical insurance cost. The unemployed population is of course one of the major factors in the uninsured crisis. This however, will never be eliminated, which is another issue in and of itself.
One thought about uninsured people is make available a centrally located office where these people can receive care from physicians and staff who are willing to work at a reduced rate, and still give good care. There are these places in big cities. This is no help for the rural communities who suffer from costly budget cuts and no powerhouse staff who want to move to the area and set up shop.
The best way to get people to act on their own behalf, is to educate them on healthcare enrollment, cost, benefits, and try to initiate self- awareness on this matter. People will always be poor, unemployed, minority, disabled, or just lazy, let’s try to use this to our advantage in the process of community awareness and help centers for healthcare questions. There are no easy answers, but we can try as a nation to find acceptable solutions for movement in the right direction.
