Anticoagulant Safety in clinical practice
Research Aim
To review the available literature on analyzing patterns, trends and issues linked with the anticoagulant-related incidents followed by a comparison of the acquired data with the clinical incidents reported by the Addenbrookes Hospital at Cambridge. Recommendations will be designed for safer practice and medication safety.
Objectives
1. Determination of most common medication errors during the administration of anticoagulants.
2. Estimation of the intensity and clinical impact of medication errors associated with the administration of anticoagulants
3. Forumulation of appealing recommendations to update the standard operating procedures and practices in UK-wide medical settings
Clinical incidents caused by inappropriate use of anticoagulants are quite prevalent as per the statistics provided by the NHS, NICE, JCI, and NPSA. Researcher of this study has, therefore, focused on this issue. During this research, inappropriate practices or commonly made errors related to prescription, dispensing, administration and monitoring of anticoagulants will be identified. Since anticoagulants are regarded as high-alert medications and efficient use of anticoagulants can lead to increased survival rates and promotion of optimal health (National Patient Safety Agency, 2004), this study is of crucial importance for the clinical practice. In particular, it is anticipated that findings of this research can be helpful in preventing adverse events caused by inappropriate use of anticoagulants.

My supervisor asked me to do a frame analysis for this data report. I did some sample of the tables, if you have additional table which could enhance the result please make it. Don’t forget to add different kinds of graphs and paragraphs for each table.
