Materials and Methods

• Clear explain How did you do to analyse the data and formulate recommendations.
The methodology must be clear and explain by details what I did in my project and could some body else at another university or hospital do this study without speak to me, just copy my methodology and do the same what I have done and they must come up with the same my result).
I will give you my idea but please don’t use my arrangement here because I give you my idea without arrangement. Use the normal arrangement for the methodology in the article. It is better to check if there any article did like what I did in my project and do the same arrangement and idea ( please check the methodology of Fanikos et al as example if you think it is good methodology)
Make a brief introduction for this chapter to explain it and indicate the summaries of the aim and objective of this study( see chapter one of my aim and objective. I attached). Then what is the material was used in this study.
Please explain what you are did from the beginning in chapter one (the introduction) and chapter three (Result Chapter) and the chapter four (discussion chapter).
I will give you some information, which will help you in this chapter:
I received the data report ( datix report) see the attached from Cambridge University Hospitals NHS Foundation Trust by one member of patient safety in Cambridge hospital . He got the approval from the head of pharmacy and head of safety department in Cambridge hospital to access the data. The data is covering two years period from April 2013 to march 2015. It is audit, anonymous, no patient identifiable data, we don’t take patient permission because just auditing errors and data report will destroy after the end of this study.
Explain briefly how the data report is classified or designed. I Attached the Policy and procedure Incident reporting and investigation & Policy and procedure Medication error investigation and management in pharmacy which will give you clear idea about the incident reporting.
Then write (appendix 1) to add a sample of this data report in this appendix. Explain the likelihood/Consequence column and actual impact column as well according to the risk assessment by details. (appendix 2) to add the risk assessment tools in this appendix. And please add the example of reporting incident in pharmacy as( appendix 3) which available in the attached Pharmacy Error Management and Reporting-page 5.
General information:
I look to the data (datix report) what is the problem?
It is number of incidents and severity of them.
Indicate the number of incidents, number of medication and what they are?
How to analyze the data?
I sorted the data by different categories, medicinename, wards,month,severity……….etc.
What statistic I used (excel or spss soft were )
What you do? ( average or mean)
Retrospective study.
Is it qualitative or quantitative?
Or qualitative data and some quantitative in the graph?
