Unit/Course Overview
This unit of competency describes the skills and knowledge required in contributing to the care of the client with a chronic illness by performing nursing interventions that support the client’s needs and assisting them to maintain an optimal lifestyle.
For students to achieve a result of ‘competent’ they will need to demonstrate the following:

Knowledge of:

– The clinical manifestations of chronic health problems on body systems
– The physical and/or psychological impacts of a chronic health problem on activities of daily living through discussion with the client and/or family (with client consent)
– The client’s understanding of their underlying/presenting condition
– The actual and potential health issues of a client presenting with a chronic health problem through discussion of information gained from a preliminary health assessment with the appropriate members of the health care team
– A problem solving approach to assess the impact of chronic disease processes on the client and/or their family and the achievement of activities of daily living
– The available resources and support services with client/s and significant others where appropriate with the consent of the client
– Admission data for the client with a chronic health problem, for inclusion in a care plan according to organisation policy
– Ongoing clinical data for inclusion in the client’s care plan in line with organisation policy
– Information and data on the activities of daily living for the client with chronic health problem for inclusion in discharge plans
– Client discharge procedures in line with organisation policy and procedures
– Document and report changes in client condition to the appropriate health care team members
– Planning the nursing care of a client requiring extended care, including restorative/ rehabilitation care
– Community based care services for a client with a need for restorative/rehabilitation care
– Planning the nursing care of a client with cancer and/or a life-limiting illness
– Advocate for clients in health and/or community settings
– Identify and prioritise nursing interventions according to client needs as documented in predetermined care plan
– Base nursing interventions on client needs and individuality and perform with respect for client’s dignity and privacy
– Consideration of cultural and religious issues in the performance of nursing interventions
– Encourage the client and/or their family to assist in the performance of nursing interventions as able
– Physical, psychological and social needs in the performance of nursing interventions
– Nursing interventions in accordance within professional, legal, ethical and organisation requirements

Assessment

Each assessment item is briefly described below. The due dates of the assessments are provided. Students who require an extension for any assessment item need to discuss this with their teacher and have the request for extension approved using form FM-063. Answer the questions listed, using referenced answers where necessary, i.e., Question 4 would not require references. Write in sentences and ensure that each answer is at least 100 words.

2. Describe two types of treatment eg. Medications, diet.

3. Describe two nursing interventions you may carry out for the person if they were admitted to hospital under your care.

VOCATIONAL PLACEMENT AND THIRD PARTY REPORT

Clinical placement book
You are required to complete 120 hours of vocational placement in either an aged care facility or a hospital. You will be given a clinical placement booklet which is to be completed by your assigned mentor. The mentor must be a registered nurse or an endorsed enrolled nurse. Final assessment will be made by GCIT’s clinical coordinator. At no time must medications be given by the student nurse until successful completion of the calculations examination in semester two.
Third party report
In addition, an approved staff member of the health facility (Registered Nurse, Nursing Unit Manager or Director of Nursing) must sign a third party report which confirms your ability as a student to work with in the AHPRA scope of practice, and that you are able to remain professional at all times.
