- Must include pertinent medications. (include how they relate to your patients condition)
- Must include your patients abnormal lab values and or diagnostic studies relevant to disease process or condition. (include how they relate to your patients condition)
Must include the following from the nursing process
- Assessment description of the patients signs and symptoms
- Diagnosis 3 part nursing diagnosis 3 total with 1 potential if desired
- Plan including 1 goal for each dx that this is pt centered measurable and timed
- Interventions 3 per diagnosis
- Evaluation was goal met.
